Supported Employment Services 

 

Supported Employment Services are offered to individuals who are unlikely to gain employment at or above minimum wage. Because of a disability, such individuals are in need of intensive, ongoing support to perform in a competitive work environment. Supported Employment is conducted in a variety of settings, particularly at work sites in which persons without disabilities are employed. Supported Employment includes activities needed by individuals to sustain paid work. Services include adaptations, supervision and training for the individuals as needed as a result of their disabilities to maintain employment. Such activities include work-related skills required to perform the job and the ongoing support necessary to assure job retention.

Community Integration Services

 

Community Integration Services are designed to assist individuals in acquiring, retaining and improving self-help, socialization and adaptive skills necessary to reside in the community. These services and related training enable the recipient to plan for and engage in meaningful adult domestic and leisure activities. Community integration can include cues, on-site modeling of behavior and/or supervision to assist the participant in maintaining maximum independent functioning.

• Money management
• Interpersonal communication
• Socialization and the development of relationships
• Development of decision making skills
• Increasing self-esteem
• Civil rights and protections
• Transportation

Service Coordination Services


Service Coordination Services will assist individuals who receive Waiver services in gaining access to needed Waiver and other state plan programs, as well as medical, social, educational and other components regardless of the funding source to which access is gained. Service Coordination is working with and at the direction of the participant whenever possible to identify, coordinate and facilitate Waiver services. Service Coordination also includes intake, needs assessment, advocacy for services from local resources and expertise in coordination of services to achieve maximum participant input and support.

 
Daily Living Services


Daily Living Services include assistance with eating, bathing, dressing, personal hygiene and other activities of daily living. When specified, this service may also include such housekeeping chores as bed-making, dusting, vacuuming and assistance with meal preparation, as it is essential to the health and welfare of the individual. This service may also include routine wellness services to enable adequate nutrition, exercise, making and keeping medical appointments and taking medications. Assistance with health maintenance activities, such as bowel and bladder routines, ostomy,catheter and wound care may also be provided.

Respite Services 

 

Respite Services are provided to individuals on a short-term basis in the event of absence or need for relief of those persons normally providing the care. Respite services may be provided in an individual's home, place of residence, Medicaid certifi ed ICF/ORC, licensed respite facility, or other community care residential facility approved by the state. Respite services may include overnight care and services which a family member or other would perform if they were present. This may include bathing, dressing, housekeeping and participating in community activities.
